Not to be confused with a security violation, a security breach is an external act that bypasses or contravenes security policies, practices or procedures. Every security breach begins with a security incident; only if confirmed does it become a security breach. Breaches can occur in a number of ways, including computer files being hacked, a backup tape with sensitive information being lost during its shipment, or a laptop being stolen from back of an employee's car.
