IT/business alignment is a perenially popular topic among IT analysts. And alignment has prompted a fair amount of soul-searching among CIOs.
But the need to cut IT costs, precipitated by the harsh economy, knocked it from the No. 1 spot on this year's list of top CIO concerns, compiled by the Society for Information Management (SIM). I'm not sure about 2007, but alignment also topped SIM's list in 2006.

It's good to see concerns about overall business productivity and cost reduction, business agility and business processes topping IT-specific concerns. It's not so good to see CIOs worrying about their own role.

...or maybe it is good to see CIOs concerned about their roles - there's so much discussion around IT being out of touch with businesses' overall strategies (as you mention, a top item on these lists for years running) - if they're worrying about how to become more effective in defining and driving to the needs of the business, this could be a good thing!
